These are the Basic Requirements for Board Exam or PRC Licensure Exam :
1. Original and Photocopy of Birth Certificate in NSO Security Paper and Birsth Certificate from LCR.
2. Original and Photocopy of Marriage Contract in NSO Security Paper and Marriage Certificate from LCR (for Married Female Applicants)
3. Original and Photocopy of Transcript of Records (Special Order No., Date of Graduation, wtih scanned pictures, additional remarks, and must be indicated with "For Boar Exam Prurpose".)
4.School Diploma.
5. Four Passport size colored pictures with complete name tag and in white background.
6. Community tax Certificate (Cedula)
7.1 Window mailing Envelope w/ metered stamp
8. Exam Fee - 900 (Baccalaureate), P600 (non-Baccalaureate)
